     IMPORTANT NOTES:

1 ) This Form is under testing, and may generate errors,

     so until customer calls Winks and gets booked for

     event, Winks reserves the right to fix and correct

     errors, and then submit corrections to customer for

     approval.

2 ) Click on all options or you may miss a discount.

3 ) Events require 3 hours minimum of music.

4 ) If you choose client picks dinner or dance music,

     each hour of music takes almost 20 songs, so

     2 hours of music would take 40 songs.

     That list must be email to info@djwink.com 1 weeks

     before event or discounts will not be given.

     Winks Karaoke & DJ Service will send an email

     reminder, 2 weeks before event.

5 ) Price is based on 2 hrs Dinner & 2 hrs Dance

     with additional Dinner Music $30 an hour and

     with additional Dance Music $50 an hour.

     Choosing less than 2 hours for either will be at

     a reduced rate.
